Guilhem f86ca0ef4f refactor: simplify group description to note, controlled by top-level Notes toggle ...

Rename "description" to "note" across schema, types, and UI components.
Remove per-group show/hide toggle and description_collapsed_by_default
persistence — group notes are now controlled by the existing top-level
"Notes" toggle. Add "+add note" / "remove note" buttons in the group
action bar. The Notes toggle also appears when any group has a note set.
